Apache2.0 - Auto vhost ads?

Hey guys,
I've been running my own servers 10+ years. I've always been into gaming and hosted all my sites and friends.

I started providing hosting services for the general public back in 2005.

My question is,
How do I include an ad across top of the pages within a vhost?
I know I could create a framed index but I was wondering how I do it on a vhost and the client cannot remove it?
