Infinite Loop when adding a .301 to index.htm

Please tell my developer the correct way to address this ...

I have read the other threads about this on the forum, but my developer is adamant there is no 'fix' for the infinite loop scenario when adding a .301 to www.example.com/index.htm

What I want to do is permanently redirect each of the following three URLs to the root - http://www.example.com/

http://www.example.com/index.htm
http://example.com/
http://example.com/index.htm

Can someone please confirm with absolute certainty the best way to do this whilst avoiding the infinite loop scenario.

Does this [google.com] help?

It's not an obvious solution, so most folks don't know about it.
