.htaccess rewrite please help

Hi there - I have great respect for those .htaccess rewrite gurus out there and was wondering if I could trouble you with helping me with the following:

I want to rewrite URL's from (and I have many subdomains):

http://sandiego.example.com/
to
http://example.com/UnitedStates/SanDiego-CA/

OR

http://london.example.com/
to
http://example.com/England/London/

OR

http://atlanta.example.com/
to
http://example.com/UnitedStates/Atlanta-GA/

Please help .. I have tried many avenues, but they just don't work.
