How to Ban Proxy IP's

how to ban proxy ip's when abusing my site

I having problems with a person who is spamming my form-mail by using proxy IP's. I've received 40+ emails in the last 2 days from one person. I found some of the IP's listed in open proxy lists.

I have no clue where to begin. How does one prevent or report this? Any ideas?
