tracking an enquiry form w/ analytics

use ecommerce analytics to track fields in forms

I want to track an enquiry form using analytics ecommerce code option. I want to generate a uniques number for each enquiry. Then i want analytics to track these codes. I know in ecommerce you can track unique order numbers so basicly this will be the same idea except i will be tracking an enquiry instead of a sale.

Will the following codes below help me do that and can some one take me step by step on how to do this. Is it as simple as adding the following code below to the thank you page.

The form will have the following:

Name: - Will not track due to Google privacy T&C
Email: Will not track due to Google privacy T&C
City: I want to track ( im sure google will not mind)
Service: I want to track
Order number: I want to track

Taking the information above can some one take me through how to place a code on the thank you page assuming basic ecommerce analytics is set up.
