Have you run your Sitemap.xml file through an XML validator?

sitemaps can be fragile. one typo can cause havoc. i'd follow tedster's advice and head over to a sitemap validator.

Just ran it through a validator and it's showing the same error -
Error: Element name image:image has unbound prefix
in unnamed entity at line 17 char 14 of http://www.example.com/sitemap.xml
Line 17 is where the first <image:image> tag begins.

This has been resolved now.
I double checked everything and found that i didn't include this code in the urlset tag "xmlns:image="http://www.google.com/schemas/sitemap-image/1.1"
