Then there's the problem with ads defrauding the visitors which Adsense apparently has no problem with, like all those software download sites with ads with big "DOWNLOAD" logos (and no line borders), when the actual link to download the file you want is usually some much smaller text link.

Different issue, and doesn't excuse it. I'm an advertiser, and a legitimate one, and I have NO sympathy for people who click their own ads, and I don't think they should be let back in, ever.

You could try to reapply for another adsense a/c (preferably in your wife's or mother's name)with your safest site and see how you go. I know a similar circumstance where that worked.

Where the publisher admittedly clicked on his own ads?
