I think the "loop holes" some of us think exist that let some advertisers slip through are most likely related to site targeted campaigns

It's not a case of us thinking it exists. It DOES exist. Nothing to do with site targetted campaigns either. These are all "big boy" sites that were filtered out, but are now bypassing filters.
