IMO accounts are mostly banned by Google's program which checks for unusual activity. Looks like it may still ban you even if you emailed AdSense support and had a "go" from AdSense support (because program is not aware of your emails).
But appeals are processed by real people. Use the link you posted for appeal. Put a note you had a "go" (include reference # from the "go" email), and they may reinstate your account.
Note: it may take few weeks or even longer (i mean their first answer).
