I'd guess a combination of everything...

I logged into to my personal account at my work PC then, foolishly, logged into the work account. (which does not belong to me) The company account was then banned, even though it has nothing to do with my own personal account.

I've since removed all toolbars, blocked cookies from G, use Tor and never log into my old accounts. Probably missing something but it seems to have worked.
