I think the key distinction is between "cookie-cutter" doorway pages, i.e., groups of vitually identical pages targeted at different keywords, and optimized content pages. If you say "doorway page" to many people, the first thing that pops into their head are auto-generated pages that all look alike except for the target words. Most SEs would consider these to be spam.

A content page optimized for a particular keyword/phrase may serve as a doorway page but wouldn't be considered spam.
