The requests for www.mydomain.com were going to the default website because www.mydomain.com wasn't in the ID list. This is Bill Gates' fault for making everything so darn complicated.
OK sorry, please ignore my post. I realize that I needed two entries in the "advanced website identification" list. One for mydomain.com and one for www.mydomain.com.
