The wordpress site title under setting tab is getting indexed by being appended to the page title + Website Title

The usual page titles that i had set up in seo plugins are not getting indexed...

I have read that google sometimes re-writes title, but does it re write every title of every page... or is this a bug which I am facing..

My website has gone down in traffic in latest update (yesterday) and I had altered site title slightly few days back...

Are the two events related?
