On the subject of the parth variants...I suspect you don't need to spell out index pages and the www variants. Google has bragged about being very good with their internal canonicalization lately. So they probably know example.com, www.example.com, www.example.com/, example.com/index.html are one and the same. Logically, if they are reusing the nofollow algorithm (which it sounds like they are), then it would make sense that they have this covered. I mean who worries about whether people link to which (www).domain.com(/)(index) variant with normal anchor links?
Regardless, google should clarify this to keep their databases from being swamped with duplicate entries.
