I'm gonna one-up the OP on the re-write. I have a website title as follows:

keyword 1 | keyword 2 | keyword 3

No matter how you search, the title always shows in the SERP as:

keyword 1

No brand name, nothing, just "keyword 1". It's been like that for months.
