I think that for a computer it is a quite easy to spot whether page title element, h1, link anchor, link title attribute and URL align just because the user is non-SEO aware who is using CMS with these "SEO features" or whether the site is over-optimised by SEO (person).

In the first case - the alignment between these elements will be the same across the site. In the second case you are more likely to have internal anchor text being subset phrase of the whole page title element and URL of over-optimised page often dropping off stop-words from the URL (which would be there if the URL is created from the CMS page name purely by replacing spaces by dashes or similar)
