Page is a not externally linkable

I would appreciate your thoughts on this as this is on a couple of sites that I am working on.
I read that if you have an internal link going from Page B to Page D in the body text, for example, and the anchor text used is build your abs and the title tag for Page D is build your abs, that is considered over-optimization.
Has anyone seen this or heard about it?
Could this be seen as too focused?
Also, if this is something that is done for several keywords (between different pages on the site but in the way mentioned above), what kind of effect can it have?
On one site that I am working on, about 20 pages, this pattern is there for over 50% of the pages. When I did this, I did not think this was over-optimization, I thought it helped to strengthen the themes of the pages, but now I am not sure what this is considered.
