I recently found a client's site (a Christian ministry) with another site (adult nudist site) as the title. After notifying Google of this mixup it was fixed but then a week later another title appeared on this site. After further checking I noticed the client's site was on shared hosting-the same IP. Thus Google was mixing up the titles. I upgraded to a dedicated IP and problem is solved.
