Keep sending it every few weeks. Make sure you acknowledge what you think may have caused the ban, and what you've done to correct it. It took me three tries over maybe six weeks to get a client site back into the index after 26 months of being completely banned - and after telling me initially that the site was indeed banned, Google never did respond - the site just showed up in the index one day. Took another three months to get any kind of SERP positions, and now it's doing well.

This is for a site that was totally banned with no pages in the index. If your problem is that you're indexed but not ranking, I'm not sure a reinclusion request is going to do any good.

And as I always say, it never hurts to ask someone else to take an objective look at your site to see if a fresh pair of eyeballs can spot something you don't.
