I have only seen it happen with new content. However, that content can be from a site that is already included.

When it shows up, it ranks very well. But those initial rankings aren't an indication where the pages will end up ranking after their PR has been caculated.

It's really more like Yahoo's new site tag. While you have it, you are listed at the top. Once it expires, you end up mixed in with all the other listings.
