I wouldn't say "usually", but a high percentage of International orders fail AVS, especially in Great Britain, where there is a huge disconnect between a Royal-Mail-standardized address, and the address customers provide when they place an order.
Regarding CVV, this verification usually works, and we won't accept an order without it.

The biggest fraud-prevention technique you can employ is to simply not ship to 170 out of the 192 countries where fraud prevails. If you do not ship to any country in Africa, the Middle East, or the Eastern Bloc, you will eliminate 95% of fraudulent international orders. Exceptions to the rule exist, but if Ghana is in your country drop-down during Checkout, you are asking for trouble. Only ship to the 20-25 lawful countries that use credit cards (and whose mail system won't drop your package in the trash), and your selection of a Gateway becomes secondary.
The gateways are all hitting the same databases for AVS and CVV, and while their mechanical fraud-filters are better than nothing, you still need to perform a manual smell-test before you ship.
