On the SPAM solution I have setup for our orginzation the three MAIN pitfalls are:

-Invalid headers
-Sender is not verfiable at the domain level (Unqualified domain)
-Graphic tags (<img=)

On the last point that is what gets most newsletters quarantiend here.

I did implement a nice feature though I set HTML filters not to delete, just quarantine. This helps our end users make their own decesions. This however is a duble edged sword as sometimes end uses make silly decesions.

Out of the box our solution came with over 14,000 "rules" that we had to balance.
