Marcia, the form is a standard maito JavaScript that employs the user's email client. For those that use a web based email service, I alternatively supply a .gif image of my email address. In any case, I screen the links for reciprocation and appropriateness before adding them. I do keep email templates for "Your link was added" or "Your link was refused." The refusal template has several reasons why the link may have been refused, which I manually check off as appropriate. It sounds like more work than it really is since I generally get only 3 or 4 link submissions per month.
