the right mime type for .ico files is 'image/x-icon'. to get shure everything works without probs, teach your server to return this mimetype by adding it to it. for apache, this can be easily done in .conf or .htaccess by the following line:
AddType image/x-icon .ico

for ie in general favicon is only displayed when the site is in the bookmarks and the icon cached.

to spoil browser using favicons, use a high color icon with really crazy dimensions like 64.000 x 64.000. ;)

a tip: quite a good ico-editor is axicons or microangelo (both win32)
