Excellent topic!

Personally, as long as I have ensured that I have done my part in enabling accessibility, I really don't go further.

I ensure that text can be resized ... but I don't provide tools for doing so. No little "[A] [A] [A]" on my site. (The only exception to this "rule" is sites that require registration and subsequent login. I typically offer font size (and style) as part of the user defined settings.)

I also ensure that if text is resized, that the layout doesn't break.

Other than that -- it's all general usability and accessibility. Clean markup. Intuitive layout. Usability features such as "skip links".
