i've encountered these numerous times. If it happens only occasionally, it's a process using more memory than the server allows - most likely a bug or misconfiguration. (For example, if the fault was with the php executable, it could be a memory leak in a script)
If it happens constant, it could be a faulty hardware issue. Do you know what process is child pid 11919?
