how to create a 5 point rating slider for an online survey form

5 Rating Slider for online survey form - I need to create a slider like this in JS/JQuery [filamentgroup.com ] for survey rating questionnaire.
please help me with the code for doing the similar thing with the JS/JQuery.
Need to complete this urgently.

One more sample is here - Javascript Sliders - www.pagecolumn.com but it uses the solid colors for sliding and 1-100 rating scale n i have to use the custom image and 5 point rating options not 1 to 100 scale like in first example.

I knw there is a slider plugin also in JQuery but how to customize it for a 5 rating scale just in case used. Pls respond.

Simply change the min, max and steps in the jquery ui slider widget:

min: 1,
max: 5,
step: 1,

But is it customizable? I mean can we use an png/gif arrow image for sliding and some other straight line image as a sliding base to look it visually rich. - Vicky....

Anything can be customized ;) jQuery UI always has a Theming tab that you can review. I would start there.

BTW I need this to work on an ipad. Any pointers? How we make sure that my stuff will work on an ipad I don't have one to test it. I guess if the site is designed with 960px width it may render well on an ipad ~ Vicky
