Hello,
I hope this is the right segment for this posting.

I am searching for a calendar to use on a form like this page (next to the "preferred appointment date" blank (center midway down).

The size is correct - small.

I have used Google in the past as a full web page. But that won't do in this case.

I had no luck in my search, perhaps my verbiage is wrong. Thanks for any help.

There are a number of "calendar" and "datepicker" plug-ins that use the jquery javascript library. Have you checked into some of those?

the Yahoo! Developer Network [developer.yahoo.com] has many such widgets available as well.
