A) The site is large enough to have permission from Google to go crazy with AdSense. B) Google is not aware of the site having more than 3 ad blocks.

I also have seen many sites which have more than 3 ads on a single page specially news sites. I think google provides this facility to those sites which have more traffic.

(E) There are invisible frames or iframes, so what looks like one page is really multiple pages.

| I suspect they are accepting more accounts as premium accounts these days. Six months ago I was talking with a Googler and asked what was needed to become a premium publisher. He said 'let me take a look at your account' and then came back and said that I shouldn't worry about becoming a premium publisher. I wasn't even close to qualifying. Then 2 months ago I got a call from my new account rep. When I asked why I now had an account rep, she said it was because I was a premium publisher. Go figure.
