When you look in source code you have Description metatag and what is on it?
Put unique titles and descriptions for each page.

I have unique Description meta tag for all page and the syntax is <meta name="description" content="xyz" />

Are the phrases in your queries included in the descriptions of the ranking pages?

Are you sure that you haven't blocked the bot, and it is only showing you as indexed?
(When Google is forbidden to crawl a site but is still finding links to it, it may show an url-only entry on queries that look relevant based on the url and the nature of the links)
Check your robots.txt file and look in your logs to see if Google has been crawling recently.
