I found few of mine main pages got page rank #4 but the Meta Description is missing in SERP and showing default title for all pages. Because of this i am loosing my traffic. Can any one suggest or guide me how to rectify.

When you look in source code you have Description metatag and what is on it? Put unique titles and descriptions for each page.

Are you sure that you haven't blocked the bot, and it is only showing you as indexed? (When Google is forbidden to crawl a site but is still finding links to it, it may show an url-only entry on queries that look relevant based on the url and the nature of the links) Check your robots.txt file and look in your logs to see if Google has been crawling recently.
