What does it mean when I do a search oke green widgets and I come in #1 on most
countries search like google.ca or .de or co.jp but on .com on 2nd page in webstertools it show #1 but its not there.
On all datercenters #1 or 2 but not .com search its been like this for months.

Looks like a manual change just for US market? Server is in US by the way.

It does seem suggestive, doesn't it? As other threads here show, it's not clear how the WMT position reports relate to all Google properties right now. How is your actual traffic from google.com on this search term?

Traffic is ok but I am sure it would be much higher if it would come up #1 on google.com

GWM sometimes shows unsecured keyword position. I have also experienced, my keyword is in #1 page (SERP) but in the live search, it shows in 17th position. So, don't worry about it, it happens sometime.

If your site is region specific, then it's fine. But in google.com it is always challenging to get the rank for your targeted keyword and you have to spend a lot of time and effort to do it.
