Requirments Document

Hello,

Does anyone know where I can find a programming requirments document template? My problem is that I often start a project with no real requirments from the clients (other than a design layout) and once the project starts (after I quote it, off course) I learn more about what they really wanted. My clients can never really communicate the real needs of the application prior to the start of project.

I'd like a document that they can use to draft up the requirments. This will serve as the official requirments document for the project - and both sides will be fully aware of the delievarables.

Any idea where I can find such a thing?

-R
