We've rewriteen out site and I simply want to redirect all of the old .htm pages (using mod rewrite) to index.php. How would I do this?

It's always a bad idea to redirect to a named index filename in a URL. The canonical URL for an index page ends with a trailing slash, and does not include the index filename.
