htaccess rewrite to change capitalization?

Does anyone know how to globally rewrite these URLs:

http://example.com/subfolder
http://example.com/Subfolder
http://example.com/subfolder/

all to:

http://example.com/Subfolder/

For some reason I get a 404 on the first three but the last one works fine. I don't have access to server config to try to figure it out and my host is not likely to help out.
